Advance Health Care Directive

WHAT IS?

An Advance Health Care Directive (AHCD) is a legal document that allows you to express your wishes regarding your medical care in the event that you are unable to make decisions for yourself. It typically includes two components:

  1. Living Will: This document outlines your preferences regarding life-sustaining treatments, such as artificial respiration, feeding tubes, and dialysis. You can specify whether you want these treatments to be provided or withheld under certain circumstances.
  2. Durable Power of Attorney for Health Care: This document appoints a trusted individual, known as your health care agent, to make medical decisions on your behalf if you are unable to do so. Your health care agent will follow the instructions outlined in your living will, but they also have the authority to make decisions based on what they believe is in your best interests.
